Understanding PrEP and HIV Prevention
PrEP, or pre-exposure prophylaxis, is an oral medication for preventing HIV infection in individuals at risk. The primary drugs used for PrEP is a type of HIV medicine. It helps prevent the virus from spreading.
PrEP works by preventing HIV from establishing a permanent infection in the body by interfering with HIV replication. Taken daily, PrEP reduces the risk of acquiring HIV through sexual intercourse by up to 99%.
Ideal Candidates for PrEP
HIV PrEP is recommended for individuals who are at high risk of acquiring HIV. This includes, but is not limited to:
-
Men who have sex with men (MSM) because of the high prevalence of HIV in this group.
-
People in relationships with differing HIV statuses: one partner is HIV-positive, while the other is HIV-negative. PrEP can be beneficial for the HIV-negative partner to prevent transmission.
-
Individuals engaging in unprotected sexual activity. This is especially true for those with multiple partners or in areas with high HIV rates. These individuals may benefit from PrEP.
-
Injection drug users (IDUs). Sharing needles or injection equipment can increase the risk of HIV transmission. PrEP can be beneficial for individuals who engage in injection drug use.
-
Commercial sex workers especially without consistent condom use.
-
Individuals with a history of sexually transmitted infections (STIs): Having an STI increases susceptibility to HIV infection.
It is crucial to remember that PrEP should be started under the guidance of a doctor. They can assess your risk factors and provide the appropriate advice. PrEP works best when taken regularly as directed. You should also use it with other prevention methods, like condoms and regular testing for HIV and other STIs.

Can I get PrEP over the counter in Thailand?
No, PrEP is not available over the counter in Thailand. PrEP is a prescription medication that you can get from a healthcare provider after proper evaluation.

What is On-Demand PrEP?
On-Demand PrEP refers to taking PrEP medication only when planning to engage in risky sexual activity. The regimen involves taking 2 pills 2 to 24 hours before exposure, then 1 pill every 24 hours, and the last pill 48 hours after the final exposure.
What are the limitations of On-Demand PrEP?
-
Research supports the efficacy of HIV prevention specifically in men who have sex with men. For female or transgender individuals, PrEP should be taken using the standard method.
-
People with hepatitis B should avoid using on-demand PrEP, as starting and stopping the medication can reactivate the virus and cause hepatitis flare-ups.
-
Taking on-demand PrEP requires careful planning and consistency, so it’s not ideal for people at high risk or unsure about taking it regularly.
-
The US Centers for Disease Control (CDC) and the Thai HIV prevention guidelines do not recommend on-demand PrEP as a primary method. Daily PrEP has more research support for effectiveness.
Do You Need to Use Condoms While Taking PrEP?
Since PrEP doesn’t protect against other STIs, it’s recommended to use condoms during high-risk sexual activities.
